I'm going to explain exactly what that means. And what, based on everything I've seen in nearly two decades, has to happen for the burning to actually stop.

Most people have never heard the cellular explanation behind peripheral neuropathy.
Your small-fiber nerves are highly energy-demanding and rely on a constant supply of ATP—the energy currency that keeps your cells functioning.
They fire distress signals.
Burning. Tingling. The deep ache that arrives in the evening. The sensation that wakes you at two in the morning. The numbness that alternates with pain in a pattern that seems to make no sense. That is your nervous system signaling an energy shortage, not structural catastrophe.

In many of the patients I see, the nerve fibers are intact. They've simply been starved of what they need to function.
Almost no one asks the next question: Why are the nerve cells not getting enough energy in the first place?
In the vast majority of patients I treat, the answer sits one critical step upstream from the nerve fiber itself. It is a delivery failure. And understanding the delivery system is what changes everything.

Take a look at the following picture.
Your lower leg has a powerful mechanism that drives blood back toward your heart:
the calf muscle pump.
Each time you walk, your calf muscles compress deep veins, pushing blood upward against gravity. One-way valves keep it moving in the right direction. This circulation delivers oxygen to your tissues, where mitochondria use it to produce the ATP your nerves need to function.

Think of it like recharging the batteries inside the cells of your leg. Every contraction is a charge. Every hour you sit still is time the batteries are draining.
When this pump slows, through reduced daily activity, more time sitting, or the gradual changes that come with age, blood begins pooling in the feet and ankles instead of returning upward.

Your ankles swell by evening. Your legs drag by mid-afternoon. And critically: the tissue in your lower leg stops receiving the oxygenated blood it needs to keep nerve cells running.
Mitochondria without adequate oxygen cannot produce adequate ATP.
Nerves without adequate ATP begin broadcasting distress signals.

This is real. I want to be clear about that.
And for a significant number of my patients, what's actually driving it is not permanent nerve damage. It is a pump that has slowed down, and a circulation system that has stopped keeping up.
A delivery problem is addressable. But only if you address it at the right depth.

Now the pattern you've lived through starts to make sense.

Compression socks. reduce swelling by applying external pressure. But they don't activate the calf pump, restore nerve oxygen delivery, or affect ATP production. So the burning and tingling can continue—despite the squeeze.

Supplements. Alpha-lipoic acid, B12, magnesium, and benfotiamine may support nerve function, but they don't necessarily address impaired circulation. Foot circulation plates and massagers.
These devices stimulate the feet, but often do little to engage the calf muscles—the main driver of venous return.

Red light at exactly 660 nanometers is absorbed by cytochrome c oxidase, the terminal enzyme in the mitochondrial electron transport chain. This is the ignition switch for cellular energy production. When it absorbs red light at this specific wavelength, it upregulates ATP synthesis. Cells that have been running on energy deficit start producing fuel again.

It is a documented metabolic response, studied since the late 1980s, when NASA researchers noticed that wounds healed measurably faster under the red LED panels they were using to grow plants in zero gravity. That observation led to eight formal research contracts and, eventually, to the science underlying modern photobiomodulation therapy.
In 2026, Scientific American reported: "a growing body of legitimate science... clinical studies have reported improvements in peripheral neuropathy."
That same year, the US FDA cleared a red-light device for the treatment of diabetic nerve pain in the feet, the first clearance of its kind in this category.

Near-infrared at 850 nanometers penetrates 2–3 centimeters beneath the skin surface, past the superficial capillaries, into the muscle and nerve tissue where the burning signal originates.
Red light alone does not reach this depth. This is why most consumer red-light devices produce only partial results for neuropathy: they deliver red light without near-infrared, and the nerve tissue itself remains untouched.
Randomized controlled trials using near-infrared specifically for peripheral neuropathy, sham-controlled, 60–70 subjects, documented statistically significant reductions in validated neuropathy scores at 4, 8, and 16 weeks post-treatment, with p-values below 0.001 at every measured timepoint.
Near-infrared reaches Layer Two: the nerve fibers and deep tissue where the burning actually lives. The layer most treatments never find.

EMS does something the other two cannot.
It triggers the calf muscle to contract, precisely, electrically, exactly as walking does.
That contraction compresses the deep veins. Blood that has been pooling in the feet and ankles is forced upward through the one-way valve system, back toward the heart. The second heart that has been quietly slowing down begins to beat again.
